Reports To: Assistant Director of Nursing
This position is non-exempt and is eligible for overtime pay for hours worked in excess of 40 during a single calendar week (Monday - Sunday).
JOB DESCRIPTION
Summary/Objective
Under the Supervision of the Assistant Director of Nursing, the Licensed Practical Nurse (LPN) is responsible for the overall oversight and supervision of the healthcare needs of the people we support in a residential setting. The LPN will work in conjunction with the Assistant Director of Nursing, Registered Nurse and the Residential Administrative team to ensure the delivery of the highest quality of integrated health and continuity of care to the people residing in our residences. The qualified candidate must be able to work in partnership with all personnel within the organization to ensure the highest quality of clinical services for the people we support in a manner that is consistent and aligned with the agency mission and vision, and the philosophy of the Council on Quality and Leadership (CQL).
Essential Job Functions
The Licensed Practical Nurse serves as a member of the Interdisciplinary Team and primary functions include:
- Perform delegated nursing functions within the scope of practice of an LPN under OPWDD (See attached guidance) and as per established procedures, policies, guidelines and standards as observed by the Registered Nurse.
- Assist in data collection on individuals within assigned caseload including, but not limited to, lab results, diagnostic testing, BM data.
- Perform tasks and responsibilities related to individual’s medical care and environment in a safe manner.
- Contribute to the development of individual’s plans and assist in identifying areas of need.
- Adhere to physician orders accurately.
- Assist in observation and evaluation of nursing care administered by Direct Support Professional Team Members.
- Document pertinent observation and intervention as required by policy and regulation: including, but not limited to, AMAP observations, BM protocols, feeding protocols, diabetic protocols.
- Assist in maintaining effective communication with staff, physicians and Registered Nurse.
- Communicate information regarding individual’s medical status and overall needs to the Registered Nurse, physicians and pertinent team members.
- Attend medical appointments when necessary and/or requested by the Supervisor.
- Review physician summary forms to ensure information is complete, accurate, and documented in the individual’s medical record, including any indicated test results and recommendations for follow up as well as resolution of a medical issue.
- Work in partnership with the program management to ensure that provider visits are followed upon and staff are securing follow up appointments as indicated.
- Ensure Emergency Room instructions and/or treatment is followed up and documented accordingly. Ensure staff are educated as needed on proper follow up and treatment. Emergency Room Documentation should be obtained by staff upon discharge.
- Provide staff trainings as delegated and observed by the Registered Nurse.
- Perform additional responsibilities, as assigned.
- Ensure compliance with regulatory standards.
- Observe agency policy and procedures regarding facility security and proper identification of authorized personnel.
- Ensure all individual’s self-preservation through implementation of the fire evacuation plan when participating in scheduled fire drills.
- Ensure appropriate protective and preventative measures are followed by staff and individuals in the control of communicable and infectious diseases along with the identification, assessment, and reporting of the same.
- Attend Inter-Disciplinary Team Meetings in assigned caseload, division meetings and staff meetings.
Duties and Responsibilities
The Licensed Practical Nurse will:
- Adhere to a flexible schedule that reflects, and is based on, the needs of the program. If warranted, change in program location and oversight may occur.
- Ensure adequate supervision is provided to the individuals based on Individualized Service Plans, Behavior Support Plans, and Nursing Care Plans.
- Ensure communication to RN, management, and direct support professionals any change in individual’s status and/or needs observed during shift.
- Ensure there is an adequate supply of medications on hand.
- Assist with ensuring accuracy of MARs for use at beginning of the month.
Education, Work Experience, and Competencies
- NYS LPN License.
- Experience working with the differently-abled population preferred
- Knowledge of OPWDD state and federal regulations preferred.
- Ability to manage multiple priorities and ability to be flexible to meet the needs of a diverse population.
- Strong verbal and written communication and organizational skills.
- Valid NYS Driver’s License.
