Job Objective
The Hospice CNA will care for clients with utmost kindness wherever they reside.  This may be include in their own community home, or facilities that may include skilled nursing. memory care, or assisted living.  Under the direction of RNCM, the CNA will follow the plan of care including, but not limited to, providing assistance of activities of daily living, such as grooming, oral hygiene, toileting/incontinence care, mobility, meal prep, feeding, as necessary, med reminders/assisting with pre-filled meds, light housekeeping.  The CNA will help clients maintain dignity and comfort while compassionately meeting the clients needs on the clients own terms during their end of life journey.  The CNA will adhere to visit frequencies set by RNCM, will communicate expected visit times with client/family, will update team with any new concerns and will document visits via hospice EMR timely, according to agency policy.

Job Standards
1. Must possess current Wisconsin Home Health Aide Certification.
2. Minimum of 18 years of age
3. Ability to communicate orally. in writing and through completion of requirements in the electronic medical record
4. Ability to see, write, hear, and speak effectively.
5. Ability to travel to and from homes/facilities within the community despite weather conditions
6. Ability to maintain and safeguard entrusted confidential information.
7. Ability to safely perform all of the essential job functions without causing harm to self or others 
8. When utilizing a personal vehicle, a valid drivers license and proof of current vehicle liability insurance coverage, as well as, a driving record that complies with St. Camillus safe driving policy.
9. Ability to work independently, be able to problem solve and demonstrate critical thinking skills.
10. Willingness and ability to respect everyone you meet and treat all people with kindness. 
11. Ability to adapt to change and flex schedule.  
12. Must have strong interpersonal communication skills to promote St. Camillus Hospice and Home and Community Based services department.  
12.   Ability to actively promote CNA services according to client potential needs.
13.   Professional appearance as directed by the department dress code.
