Reports to: RN Care Coordinator

Program: 1006- SOLAR

Department: Interim Housing

Location: SOLAR - 4200 East Compton Blvd., Compton CA 90221

Setting: 100% Onsite

Schedule: Sunday, Wednesday, Saturday: 7am - 7:30pm

Status: Full-time/Non-Exempt/Non-Management

SUMMARY:

SOLAR is a DHS funded Recuperative Care Program serving individuals experiencing homelessness that also have medical issues. The LVN/LPT Advocate will take a leadership role in medication and health issues in the Recuperative Care program and will ensure the quality and adequacy of nursing services provided to members.

 

Essential Duties and Responsibilities: 

  • Complete medical assessment intakes upon admission to SOLAR.
  • Coordinate PCP appointments or other medical follow ups upon admission.
  • Provide medical case management and supportive services to individual members (caseload of 20-38).
  • Provide wound care to members as needed.
  • Document, chart and file all services provided on nurse charting forms with accuracy and in real time.
  • Provide medication management to individuals including but not limited to medication monitoring, patient teaching, and supportive services to increase compliance with medication regimens.
  • Work in collaboration with the rest of the SOLAR team- inform NP, case manager, and mental health provider, as appropriate, of any medical and/or medication concerns.
  • Maintain medication logs and medication storage in compliance with safety standards and industry standards of care.
  • Provide crisis intervention assistance when needed.
  • Be able to notify the appropriate community agency in an emergency (i.e., police, paramedics, fire department)
  • Obtain vital signs twice a day am/pm times; once a month take weights measurements, and upon intake.
  • Help transfer, lift, raise, and turn residents; utilize proper lifting and body mechanics to prevent personal injury
  • Provide one‐on‐one personal care assistance to residents as directed (grooming, feeding, and other support).
  • Assist in toileting needs of residents and empty drainage collection devices
  • Assist members to maintain their bedroom areas clean, report to member's advocate any issues
  • Assist with meal service and cleaning of kitchen and dining hall.
  • Assist with organizing linen closets, toiletries, nursing areas.
  • Maintain open communication with direct supervisor, individual and group supervisor(s).
  • Cooperatively and cohesively work with other parties involved in the member’s case.
  • Participate in bi-weekly staff meetings, bi-weekly nursing meetings, weekly individual and monthly supervision.
  • Ensure that medical services are reflective of the agency’s treatment model, which emphasizes member choice, harm reduction, trauma-informed care, and psycho-social rehabilitation.
  • Conduct Health/Wellness and medication management groups.
  • Maintain confidentiality of member files per HIPAA and all applicable guidelines.
  • Other duties as assigned.

 

Qualifications:

  • Current, California license as a Licensed Vocational Nurse with a clear license.
  • Minimum one years’ experience working with individuals experiencing chronic homelessness, mental health, and substance abuse issues.
  • Able to physically lift, move, and position members as needed.
  • Able to demonstrate acceptance toward members who may have poor coping skills and a high degree of cultural sensitivity to their needs.
  • Experience completing medication logs to accepted industry standards.
  • Proficient in Microsoft Word, Outlook, Excel, and database applications.
  • Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills.
  • Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision.
  • Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.
  • Able to obtain and maintain CPR/First Aid certification

 

Preferred Qualifications:

  • Experience working with those living with mental illness and/or substance addictions.
  • Experience completing DHS documentation.
  • Bilingual Spanish preferred.
  • Minimum 35 WPM typing speed preferred.

 

Work Environment:

  • Combination of field and office environment
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
  • On occasion walk or drive to different local sites
  • Will necessitate working in busy and loud environments
  • Will be exposed to elements like cold, heat, dust, noise and odor
  • May need to bend, stoop, twist and sit throughout the day
