POSITION SUMMARY

The Senior Community Health Outreach Coordinator engages and organizes the community in services and activities with a focus on mental health and substance use recovery. The position and provides education, outreach, referrals and support to the community as well as individuals and families served. 

Salary $63,440

EDUCATION AND EXPERIENCE/QUALIFICATIONS

BA/BS degree required or Masters degree preferred plus 1 year related experience in community organizing, social work, prevention/health promotion or other social service field is required.  Knowledge of or residence in the target community strongly preferred.

A valid driver’s license and personal vehicle and some ability in the evening and on weekends are required.

Ability to work independently and on n a team to ensure program goals are met, communicate effectively verbally and in writing, and is committed to personal and program growth. Ability to work  with funder to achieve outcomes.   Ability to manage time effectively. Ability to represent Wheeler in a professional manner including convening and facilitating community stakeholder meetings and providing staff support for community activities. Proficient in use of MS Office suite, virtual meeting platforms and creative software such as Canva..

ESSENTIAL DUTIES AND RESPONSIBILITIES

Participates in the planning, coordination, and implementation of community engagement, education, outreach and support activities designed to enhance access to behavioral health and services. Outreach and education activities include prevention, health promotion, and wellness training events and programs; behavioral health screening events; recovery support services; mutual support groups; and referrals to other community resources.

Serves as an experts in behavioral health resources and information for stakeholders and community members.

Creates and delivers comprehensive behavioral health education trainings, workshops and presentations to diverse audiences.

Convenes and provides staff support to a standing care coalition comprised of community stakeholders including behavioral health and social service providers, municipal leaders, public health officials, educators, prevention experts, community and faith-based groups, and others. Duties include convening and facilitating bi-monthly meetings, distributing meeting announcements, establishing agendas, creating minutes and announcements, and providing staff support to coalition-selected initiatives. 

Strengthens existing collaborations and establishes new partnerships and referral sources in the community. This may include, but is not limited to, schools, senior centers, food pantries, the faith-based community and other human services and community organizations.

Plans and participates in community events, committees, planning initiatives and other activities to enhance community and individual behavioral health outcomes.  Represents Wheeler’s Connecticut Center for Prevention, Wellness and Recovery in the community as needed.

Creates and disseminates an electronic and print resource directory for behavioral health services available to the target community.  The directory will include both publicly and privately funded resources such as mobile crisis teams, child guidance clinics, private practices and other resources available to meet needs across all ages and stages of life. 

Coordinates access to resources, including referral to specialty services and advocates for individuals and families as needed.

Maintains records and tracking the number of individuals engaged, referrals made, and details of events hosted..

Supports daily program specific operational functions as required.

Attends and is prepared for scheduled supervision, team and staff meetings. Seeks additional supervision or consultation as needed and follows through with supervisory directives.

Demonstrates clear, concise, professional writing and oral reporting skills.

Works effectively as a member of a multi-disciplinary team by communicating appropriately and positively with colleagues, families, internal and external service providers, administrative staff and actively preparing for and participating in team meetings.

Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
