Liberty Resources’ Integrated Health Care, a Federally Qualified Health Center and Certified Community Behavioral Health Center, is currently seeking a Practice Manager.

Position Summary:

The Practice Manager is responsible for coordinating and optimizing cross-functional operations, ensuring seamless collaboration between departments to achieve organizational goals. This role involves managing day-to-day operations, driving process improvements, and leveraging technology to enhance efficiency and performance. The ideal candidate will have strong leadership skills, an analytical mindset, and a proven ability to manage complex systems and teams.

Job Responsibilities:

 

  • Lead and support the implementation of new systems and processes.
  • Ensure operational alignment with company objectives and performance goals.
  • Monitor workflows to identify and address inefficiencies and anticipate problems.
  • Manage cross-functional teams, fostering collaboration and accountability. In collaboration with Team Leads oversee front office functions including the development of a training and orientation program.
  • Maintain a coordinated patient flow and provider schedule to maximize access and efficiency.
  • Monitor appointment volumes, visit completion rates, and no-show trends; implement improvement strategies.
  • Address patient complaints and ensure appropriate follow-up.
  • Coordinate regular staff meetings and morning huddles; establish the daily plan and work to overcome obstacles.
  • Serve as a super-user for the electronic health record (EHR).
  • Collaborate with leadership to formulate and review policies and establish procedures for the integrated practice. 
  • Oversees and ensures quality processes for patient referrals, hospital discharges, and subspecialty relationships in the community.
  • Ensure policies and best practices are up to industry standards and meet the Department of Health Article 28 and Health Resource and Services Administration (HRSA) regulations as well as Article 31 and Office of Mental Health (OMH)
  • Collaborate with clinical teams, IT, and quality on the setup and efficiency of the Electronic Health Record and Data Reporting/Outcome measurement functions.
  • Ensure appropriate staffing coverage to meet patient needs at all times; coordinate scheduling including coverage for absences, vacations, holidays, and illness.
  • Demonstrate a commitment to person-centered care; foster an environment that respects individual needs, preferences, and experiences.
  • Coordinate with the Facilities Department for plant/office projects and issues
  • Coordinate with vendors and key partners including hospitals, pharmacies, specialty/subspecialty practices, laboratory, and after-hours support.
  • Ensure all health and safety obligations are adhered to including risk assessment and accident/incident reporting; review patient satisfaction surveys and create quality improvement plans as indicated
  • Participate in divisional workgroups
  • Travel between Syracuse and Fulton sites is required
  • Other duties as assigned

 

Qualifications:

 

Bachelor’s degree in nursing, healthcare administration, public health, business, operations management, or a related field (Master’s preferred), or Licensed RN or LPN.  5+ years of experience in operations management, with at least 2 years in a leadership role.  Strong analytical and problem-solving skills.  Excellent project management and organizational abilities.  Proficiency in operational software and tools.  Exceptional communication and interpersonal skills. Experience in Primary Care and/or Behavioral Health Outpatient services is a plus.

 

Salary Range: $75,000 - $100,000+.
