What You’ll Do:
- Empower and Support Clients: Provide comprehensive, long-term case management services, guiding clients through structured life management programs that will help them overcome obstacles and achieve stability.
- Mentor and Advocate: Build trusting relationships with clients, serving as an advocate and mentor to help them access the resources they need to thrive in their environment.
- Personalized Program Development: Tailor program plans to each client’s unique needs, with a focus on sustainability and long-term success.
- Collaborate and Innovate: Work with community agencies and internal teams to ensure that clients receive holistic support, using your knowledge of local resources to guide them toward success.
What We’re Looking For:
- Experience in Outreach & Case Management: We value professionals with a background in social service case management, particularly those with hands-on experience working with diverse, vulnerable populations. Experience in the non-profit sector is a plus!
- Strong Communication Skills: Your ability to connect, communicate, and motivate others will be key to helping clients achieve their goals.
- Client-Centered Approach: You are passionate about developing and implementing individualized plans to help clients reach their full potential.
- Collaborative Spirit: You thrive in a team environment and have a history of building strong, productive relationships with clients, colleagues, and community partners.
Qualifications:
- Education: Bachelor’s degree in Social Work, Behavioral Science, or a related field.
- Experience: At least 3 years of experience providing direct case management services, including developing long-term action plans for clients in need.
- Technical Skills: Familiarity with HMIS systems and Section 8 Housing is a plus.
Working Environment:
- This role requires a mix of office and fieldwork, including community and off-site meetings.
- While the position is primarily mobile, you’ll need to be flexible in managing physical activities, such as lifting or walking.
- You’ll work in a supportive, mission-driven environment with a strong focus on meeting client needs while maintaining professional boundaries and adherence to safety protocols.
