Sequel Transition Academy MAY BE THE PLACE FOR YOU!

 

Established in 2013, Sequel Transition Academy is a step down, transition program for males age 16-20 that have completed a long-term residential program. Our campus is located 7 miles west of Sioux Falls, South Dakota. Our academy offers a year-round academic program provided by the local school district. Currently, Sequel Transition Academy serves state of South Dakota Department of Corrections commitments.

The Job

The primary function of the Case Manager is to write and prepare all initial care plans, updated service plans, progress reports (as needed) and discharge summaries in a timely manner in accordance with State of South Dakota Regulations. Maintain safety, accountability (correct count of each student) and security of campus. Coordinate with entire team regarding admission, discharge and transportation of youth. Act as a resource person from team to outside systems including referring workers, families, schools and other stakeholders. Coordinate all activities with court and referring agencies regarding admissions and discharges. Maintain an ongoing awareness of students’ behavior and progress in the program.


Snapshot of Daily Duties

  • Communicate with students Primary prior to scheduling appointments/visits, etc.
  • Filing of all forms must be put into the student files immediately upon receipt.
  • Ensure all sections are completed in student files within 30 days of a student’s admission / if information is not obtainable must report this to the Group Living Director prior to the 30 days.
  • Maintenance of student files must all South Dakota licensing requirements (Sections I, II, III, IV) located in each file.
  • Student pictures are uploaded to PANDA upon admission.
  • Consents are signed prior to admission or at admission
  • Collaboration with Group Living in facilitating and attending staffing’s with Juvenile Court Agents and families.
  • Maintain a calendar and prepare and mail notices of all staffing and court dates for students.
  • Ensure that all guidelines, with regard to written reports and correspondence are followed as required by the court, referring agency, licensing and Policy and Procedures.

 

The Must Haves

  • Bachelor’s Degree in the human service field required, demonstrated ability in professional writing or at least 4 years of relevant childcare experiences.
  • It is an essential function of the position that the staff member must be capable and proficient in the area of Safe Crisis Management when, and/if necessary
