Q/A & Case Management - Transitional Coordinator

: Coordinate with treatment team regarding transitions, participate with DJJ regional transition teams in transition. Directs the transition process in cooperation with case management team. Coordinate with youth families and community resource base on needs. Act as a resource person for all staff dealing with outside systems. Work as a contact person for outside jobs. Ensure that all guidelines are followed as written by court and referring agency. Conduct post release follow ups 1 year later. Coordinate with local education.
