Q/A & Case Management - Case Manager Transitional

Essential Functions Statements:
• Coordinate with treatment team regarding transition and release process.
• Participates with DJJ regional transition teams in transition of youth from the program.
• Directs the transition process in cooperation with case management and transition team members.
• Coordinate with youth, families, and community resources based on needs identified at the transition staffing.
• Act as a resource person for all staff dealing with outside systems (i.e. job placement, continued treatment).
• Works as a contact person for outside job agencies interested in giving second opportunities to troubled youth.
• Ensure that all guidelines (i.e.: written reports and correspondence) are followed as required by the court and the referring agency.
• Coordinate with local education / vocational providers for continued education.
• Conduct post-release follow up to one year.
Adheres to the policy and practice concerning issues of creation, editing, transmission, storage and disposition of all Protected Health Information (PHI) of Sequel Youth & Family Youth Services.
• All other duties as assigned

SKILLS & ABILITY:
Education: Bachelor’s degree in psychology, counseling or related field.
Experience: 1 year experience to adjudicated adolescents to include family and group work or equivalent combination of education and experience.
Computer Skills: Basic computer skills, including usage of email, Internet Explorer, and print documents.
Certifications &Certification is case management preferred.
Other Requirements: Fluent in the English language to interact with governing bodies, administration and the community both in person and over the phone.
