Program Overview:
Palm Beach Youth Academy is a Residential Facility for males ages 13-19. Our youths are committed by a judge throughout the state of Florida to stay in our (SAOS) Substance Abuse Overlay Services and (MHOS) Mental Health program for a period of 9-12 months. Sequel operates the program for the Department of Juvenile Justice, a level 8 high-risk secure. Our program exercises a behavior management system and provides Substance Treatment, Mental Health Counseling, Individual and Family Therapy. Our purpose is to provide interest of the youth to help frame their current strengths and attributes in a tone supportive of their employability. We are looking for a qualified Case Manager to help prepare our future men back into the community.
POSITION SUMMARY:
The Case Manager coordinates and delivers various programmatic interventions based upon strengths and needs identified in a treatment or service plan.
Education:
Must have a Bachelor’s Degree

Essential Functions Statements:
- Interviews families and students to acquire historical information to be used in developing case assessments.
- Plans and executes Family Day Schedule.
- Conducts group sessions, individual sessions and family sessions, using professionally prescribed teaching methods, techniques and documenting all services delivered.
- Provides utilization review reports to referral source and sponsoring agencies.
- Initiates family contact using professionally prescribed methods and techniques to convey and receive information regarding students during and after enrollment.
- Interacts with students in a professional manner to elicit positive change.
- Completes other reports, assessments, reviews as assigned in a thorough and timely manner.
- Supports by action and example the Sequel TSI Creed.
