Job Objective |
Accepts and carries out the Social Service programs for residents in St. Camillus Health Center. Assists residents with adjustment to institutional living; assesses resident's psycho-social needs and creates/develops and carries out plan to meet the same. Acts as a liaison and referral person for resident and family problem solving. Is a compassionate advocate of the elderly. Assists supporting admissions efforts to achieve full occupancy within the Health Center. |

Job Standards |
1. Bachelor degree in Social Work or related field from an accredited college or university. 2. Minimum of one year experience in long-term care setting. 3. Must have strong interpersonal skills, the ability to problem solve, and excellent conflict resolution abilities and written/verbal communication skills. 4. Knowledge of community resources available to residents and families. 5. Able to work independently with little supervision. 6. Ability to safely perform all essential job functions without causing harm to self or others with or without reasonable accommodation. 7. Computer skills required. 8. Must maintain current Social Worker licensure for the state of Wisconsin (CSW). |
