LSSI is hiring a full-time Occupational Therapist Monday through Friday from 9:00am to 5:00pm. Responsible for providing intensive occupational therapy services to Class Members that are part of the Comprehensive Class Member Transition Program (CCMTP) to prepare the Class Members for a safe and successful transition into independent living in the community.

Essential Functions: 

  • Provide and coordinate a continuum of treatment services, including collaborating with Care Managers, Transition staff, RNs, and others.
  • Carry a caseload of Class Members, providing a spectrum of occupational therapy services as assigned by Supervisor.
  • Create and implement individual treatment plans.
  • Meet reporting, productivity, and service deliverable requirements identified by LSSI, DHS, and funding bodies.
  • Prepare and maintain records and reports related to patient evaluation and treatment plans.
  • Provide skills that address the full scope of patient’s needs.
  • Complete electronic healthcare record (EHR) documentation in a timely manner and in compliance with DHS and Rule 140 guidelines.
  • Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
  • Maintain familiarity with Emergency Procedures.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, and other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.

Position Qualifications: 

  • Master’s degree in occupational therapy required.
  • One (1) to three (3) years’ experience in mental health field preferred.
  • OT/L (Occupational Therapist, License) required.
  • Demonstrated proficiency in EHR documentation and review required.
  • Experience adhering to accreditation and licensure standards preferred.
  • Demonstrated ability to communicate in a clear, comprehensible manner, both verbally and in writing.
  • Excellent communication, organization, presentation, and pc/computer skills, including experience using Microsoft Office (Outlook, Teams, Word, Excel, PowerPoint) along with other related software.
  • Background check clearance required.
  • Valid driver's license, in good standing for the state of residency required. Access to reliable transportation required.
  • Valid IL statutory minimum liability insurance coverage, bodily injury and property damage required.
