Provide intensive occupational therapy services to Class Members that are part of the Comprehensive Class Member Transition Program (CCMTP) to prepare the Class Members for a safe and successful transition into independent living in the community. 

Occupational therapy is a holistic, client-centered based approach to life skill development.  Creates and implements individual treatment plans.

 

ESSENTIAL FUNCTIONS:

  • Provide and coordinate a continuum of treatment services, including collaborating with Care Managers, Transition staff, RNs and others.  Operate as part of a treatment team. 
  • Carry a caseload of Class Members, providing a spectrum of occupational therapy services as assigned by Supervisor. 
  • Meets reporting, productivity, and service deliverable requirements identified by LSSI, DHS, and funding bodies.  This role requires the Occupational Therapist to  work as part of a multi-disciplinary team.  Collaborates with other LSSI programs/departments, including ACT and CST, as needed.  Prepares and maintains records and reports related to patient evaluation and treatment plans.  Provide skill that address the full scope of patient’s needs.
  • Complete EHR documentation in a timely manner and in compliance with DHS and Rule 140 guidelines. 
  • Ensure adherence to ethical standards of treatment, including NASW Code of Ethics.
  • Familiarity with Emergency Procedures.
  • Demonstrate professional, positive behavior and carry out responsibilities with integrity, treating clients, families, and other LSSI workforce members, and collaborative organizations and/or individual in a dignified, respectful, honest, and fair manner.

 

Additional Functions/Responsibilities:

  • Demonstrate sensitivity and responsiveness to cultural differences of clients, coworkers, and community.  Compassionate and a team player.   Enthusiastic and self-motivated.
  • Respond to all incoming correspondence and questions in a timely manner; including but not limited to inquiries received via email, telephone, text message, and fax.
  • Ensure reasonable measures are taken to protect confidential and/or sensitive information as well as ensuring the security of all LSSI systems is maintained.
  • Responsible for completing all required training assignments as well as complying with the related organizational, administrative, and programmatic policies.
  • Perform all other duties and tasks as assigned.

 

POSITION QUALIFICATIONS

  • Master’s degree in occupational therapy required. 

 

Professional Licensure/Certifications: 

 

  • OT/L (Occupational Therapist, License) is required.

 

Job Knowledge, Skills & Experience:

 

  • Excellent communication, organization, presentation and pc/computer skills (including proficiency with Microsoft Office – Outlook, Word, Excel and PowerPoint) along with other related software.
  • Proficiency in EHR documentation and review.
  • Familiarity with accreditation and licensure standards.
  • 1-3 years of experience in mental health field is preferred.
  • Excellent written and verbal communication skills.
