Position Summary:
The Certified Vision Rehabilitation Therapist (CVRT) carries out the vision rehabilitation goals of the agency and provides direct service to assigned consumers who are legally blind.
Job Responsibilities:
• Provides individualized services based on needs to consumers who are legally blind ensuring that all consumers receive appropriate levels of care.
• Provides counseling and support to legally blind consumers and their family members.
• Works directly with consumers to ensure that they obtain services and adaptive equipment that will maximize their functioning, potential, independence, and sense of well-being.
• Conducts ongoing assessments to identify consumer’s strengths, and barriers, and establish clear goals that directly affect the consumer’s need and goal to maintain independence. This includes linking individuals with additional support services or other community agencies.
• Maintains relationships with community service providers, low vision doctors, low vision specialists, mental health and senior services providers, as well as other service providers to ensure consumers are well supported. Establishes clear lines of communication to ensure the best quality of care is received.
• Maintains contract outcomes and completes all reporting for the NYSCB in accordance with the state contract and guidelines.
• Completes all required documentation, maintains case records and completes statistical information on time and in accordance with program standards.
• Attends and participates in department meetings and professional development opportunities as required. Maintains required program certifications.
• Train and support Blind Services Staff.
• Provides culturally competent services that empower consumers by using consumers’ cultural strengths and informal support networks in service delivery.
• Performs other duties, responsibilities and related tasks as assigned.
Qualifications:
Master’s degree in Vision Rehabilitation Therapy with one year’s experience desired.
Master’s degree in related field with two years’ experience desired.
Bachelor’s degree in Vision Rehabilitation Therapy or related field with three years’ experience desired.
AER Certification in Vision Rehabilitation Therapy with required recertification every five years required.
Ability to problem-solve in a fast-paced and complex system.
Effective verbal and written communication skills.
Travel is necessary.
Regularly required to sit, stand, walk, bend, climb stairs and lift objects of up to 10lbs as required.
Must have a valid New York State driver’s license and/or access to reliable transportation.
Hourly rate: $30.00/hour
