Bilingual Care Manager
JSSA’s Aging in Place department has an immediate opening for a full-time, bilingual (Russian-English) Care Manager. Our Aging in Place department is comprised of experienced professionals who provide full-service care management, advanced planning assistance, escorted transportation, socialization programming and kosher home-delivered meals. Additionally, JSSA is the provider of choice for Holocaust survivor services and family support.
Responsibilities:
- Provides assessments, care coordination, referrals, advocacy and access to appropriate benefits to senior in the community.
- Works closely with contacts in the community, improving service delivery for our clients.
- Carries a full caseload and will work with Holocaust survivors and seniors who are a part of a national pilot project.
Qualifications:
- Three years of experience with complex care management; preferably with an emphasis on aging
- Demonstrated experience conducting in-home assessments, providing information and referrals to eldercare resources, and working with diverse populations
- Successful candidates will be interested in working with a team
- He/she will have the ability to clearly document all client interactions
- Candidates must have reliable, independent transportation
- Must speak fluent English and Russian
