The Psychiatric Nurse Practitioner (PNP) will work collaboratively with the healthcare team which includes medical assistants, primary care providers, addiction specialists, mental health and psychiatric providers, patient care coordinators, and registered nurses to provide patients with comprehensive quality healthcare that addresses both biomedical and psychosocial needs. The PNP will provide comprehensive mental health services to our diverse patient population, including assessment, diagnosis, treatment, and follow-up care. This role requires a strong commitment to patient-centered care and collaboration with our multidisciplinary team to achieve the best outcomes for our patients. The core responsibilities include a high degree of interpersonal competence with both staff and patients and evidence- based approaches to routine psychiatric care, medication management, and chronic care support. Participates in quality improvement activities and may contribute to the practice and program development.
KEY RESPONSIBILITIES
• Provide psychiatric health care to adults and adolescents including management of common acute and chronic illness.
• Perform focused episodic history and physical exams and treat patients’ minor acute as well as urgent (non-emergent) psychiatric conditions.
• Collect a comprehensive history on all continuity patients, perform ongoing assessment of psychiatric illness, and order appropriate diagnostic tests.
• Formulate diagnoses based on interpretation of health history, physical exams, and diagnostic tests.
• Utilize all the above information in formulating and documenting health care plans which include treatment, health maintenance, education, counseling, and follow-up.
• Furnish or transmit medication orders as designated by licensure and according to clinic procedure.
• Foster collaboration and client referrals between Substance Abuse Treatment Services.
• Supports the care team in the psychiatric treatment of the patient
• Fosters and participates in integration of patient care services, including support for coordination of care.
• Conduct comprehensive psychiatric evaluations, including patient history, mental status examination, and diagnostic formulation.
• Develop, implement, and monitor individualized treatment plans based on evidence-based practices.
• Prescribe and manage psychotropic medications, including monitoring for efficacy and side effects.
• Provide psychotherapy using various modalities such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and other evidence-based approaches.
• Offer crisis intervention and management of psychiatric emergencies, including assessment and stabilization.
• Educate patients and their families about mental health conditions, treatment options, and strategies for managing symptoms.
• Collaborate with a multidisciplinary team, including psychiatrists, psychologists, social workers, and other healthcare professionals, to ensure comprehensive patient care.
• Maintain accurate and timely documentation in electronic health records (EHR) in compliance with clinic policies and regulatory standards.
• Participate in quality improvement initiatives and continuing education to stay current with best practices in psychiatric care.
• Ensure adherence to ethical standards and legal requirements in all aspects of psychiatric practice.
QUALIFICATIONS
Education, Certification, and Experience
· A valid and active California Psychiatric Nurse Practitioner license, without any limitations on license to practice in specialty.
· Master’s or Doctoral degree in Nursing from an accredited program.
· Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
· Eligible to participate in the CMS Medicaid/Medicare reimbursement programs.
· Current BLS/ACLS certificate or recertification within 2 months of hire.
· Experience working with the criminal justice population preferred.
· Minimum of 3 years of clinical experience in psychiatric/mental health nursing, preferably in an outpatient setting.
Background Clearance
· Must not be on active parole or probation, clear with OIG database.
· Ability to be able to complete a background check and live scan clearance.
Knowledge
· Culturally competent and able to work with a diverse population.
· Strong proficiency with Microsoft Office applications, specifically Word Outlook and internet applications.
· Experience working successfully with issues of substance abuse, criminal background, and other potential barriers to economic self-sufficiency.
· Knowledge of co-occurring disorders and trauma-informed treatment preferred.
· Knowledge of Clinical documentation (treatment plans, progress notes etc.) preferred.
· Knowledge of third-party payer regulations, California practice guidelines and malpractice laws, Title 22, Bureau of Primary Care, and state funding sources.
· Bilingual in county threshold languages, preferred.
