Summary


Provide a wide variety of service coordinating duties including accessing community-based services, assisting members in the development and evaluation of their needs assessments and individual plans of service to meet all requirements.

 

Engages in the work-ordered day, recreational and leisure activities to ensure members empowerment.

 

Essential Duties & Responsibilities

 

  1. Facilitate social activities, discussion groups, and outings.
  2. Function as a key staff member.
  3. Work with members on rehabilitation goal planning.
  4. Adhering to the continuous improvement of policies and procedures.
  5. Develop plan for service delivery with members.
  6. Develop rapport with members and staff.
  7. Provide program supervisor with necessary reports in a timely fashion.
  8. Provide and/or assist in member’s orientation and plan development.
  9. Provide own transportation and adequate liability insurance.
  10. Work side-by-side with Clubhouse members to support Clubhouse operations. These operations include cooking lunch daily, cleaning all areas of Clubhouse, running meetings, growing a garden, producing an in-house newscast, maintaining administrative needs, running a snack bar.
  11. Assist members in learning life skills through the daily work units.
  12. Generate member interest in participation in the work ordered day and other scheduled activities.
  13. Help members to identify and use community resources that promote their well-being, independence, satisfaction, and self-sufficiency.
  14. Assist members in seeking employment and educational opportunities in the community.
  15. Assist members with resumes, interview techniques, and applying to jobs.
  16. Provide job coaching services for members involved in all types of employment: Transitional, Supported and Independent.
  17. Maintain enthusiasm, commitment and belief in the Clubhouse model.
  18. Conduct outreach activities to referral sources to enhance membership.
  19. Participate in the admission and orientation of new members.
  20. Cover, on a rotating basis, evening, weekend, and holiday social programs. Participate in Clubhouse functions and events as needed.
  21. Involve Clubhouse members in all levels of management and decision making.
  22. Work with quality assurance to ensure compliance standards are maintained.
  23. Develop rapport with members and staff.
  1. Maintain a flexible and positive attitude.
  2.  Provide program supervisor with necessary reports in a timely fashion.
  3. Utilize Mac software including iMovie.
  4. Comply with all Agency policies, procedures and safety standards throughout work areas and maintain same to meet CARF, Department of Labor and other regulatory standards.
  5. Other duties as assigned.

Education Requirements


Bachelor’s degree plus (1) years related experience.  Qualified Mental Health Professional preferred.

 

Experience Requirements

 

One year experience working with Mental Health population.  Preferred working knowledge of MAC software including iMovie.

  2. Knowledge of recipient rights and the ability to advocate for those rights.
  3. Ability to work effectively with individual and groups both in and out of the Clubhouse setting. 
  4. Understanding of vocational services and techniques.
  5. Ability to document, record, write information accurately and thoroughly.
  6. Ability to drive 15 passenger vans. 
  7. Willingness to work flexible schedule with evenings/weekends/holidays rotation.
  8. Commitment to the community adjustment of people with serious and persistent mental illness.
  9. Annual criminal background check may be required of employees serving mental health trainees and behavioral health services programs.
