About the Program

Our Housing Support Program is designed to promote housing stability, wellness, recovery, independence, and community integration for adults with Severe and Persistent Mental Illness (SPMI) and histories of trauma. This program plays a key role in reducing hospitalizations, emergency room visits, incarcerations, and crisis episodes while increasing community engagement and long-term wellness.

Your Role

As a Housing Support Case Manager, you’ll be the frontline advocate helping individuals maintain stable housing while navigating life’s challenges. You’ll partner with clients and their support systems to coordinate care, teach life skills, and build connections to vital resources and services — all with a trauma-informed and person-centered approach.

Key Responsibilities

  • Manage a caseload of 25–30 clients with regular weekly or bi-monthly in-person check-ins
  • Assist with housing search, leasing, and move-in support, including setting up essentials like furniture and utilities
  • Develop individualized Housing Stability Support Plans in collaboration with clients and clinical teams
  • Educate tenants on rights, responsibilities, and successful landlord-tenant relationships
  • Provide early intervention for issues such as late rent, lease violations, or neighbor conflicts
  • Conduct safety and wellness checks, housing inspections, and support during housing recertification processes
  • Facilitate referrals and linkages to medical, behavioral health, entitlements, and other community-based services
  • Participate in weekly team meetings and meet monthly productivity and documentation standards

Minimum Qualifications

  • Bachelor’s degree in human services, psychology, counseling, criminal justice, sociology, social work, or related field
    (Equivalent education and experience may be considered)
  • At least 1 year of experience in behavioral health or case management
  • Strong care coordination skills with an ability to connect individuals to internal and external supports
  • Tech-savvy – comfortable using electronic health records, mobile phones, telehealth systems
  • Reliable transportation, valid Georgia driver’s license, and willingness to travel within Region 3
  • Must pass pre-employment drug screen and background check

 

Schedule & Work Conditions

  • Typical hours: Mon – Fri, 8:15 AM to 5:00 PM
  • Some evenings and weekends required for on-call support and crisis coverage
  • Work is conducted in the community, at client homes, and via telehealth
  • Travel throughout DBHDD Region 3 | Full-Time | Hybrid/In-Community Work
