We’re currently seeking a dedicated and compassionate Developmental Disabilities Professional (DDP) to join our team and help deliver person-centered, high-quality care and coordination for individuals in our community programs.

What You’ll Do

As a key member of our interdisciplinary support team, you’ll ensure each individual receives the services, supports, and advocacy they need to thrive. Your role includes leadership, documentation oversight, care coordination, and hands-on support when needed. Key Responsibilities Include:

  • Fostering a positive, high-performance team culture rooted in person-centered supports
  • Monitor staff activities and ensure timely, accurate documentation for all individuals on your caseload
  • Perform Developmental Disabilities Professional (DDP) duties and participate in ISP meetings
  • Provide coaching and feedback to team members to ensure quality service delivery
  • Act as a key point of contact and communication bridge among team members, families, and external partners
  • Monitor and respond to changes in individuals’ health, behavior, and environment
  • Ensure health, safety, and well-being by coordinating care across residential, day programs, nursing, and behavioral services
  • Provide individualized support to maintain required staff-to-client ratios
  • Lead training efforts based on identified needs during ISP meetings
  • Ensure implementation, tracking, and documentation of ISP/BSP goals and support strategies
  • Submit incident reports accurately and promptly using online systems
  • Champion each individual’s right to community membership and valued social roles

 

What We’re Looking For

Minimum Requirements:

  • Bachelor’s degree in a human services-related field
  • At least 1 year of experience supporting individuals with developmental disabilities
  • Valid Georgia Driver’s License and acceptable Motor Vehicle Report (MVR)
  • Basic computer skills and the ability to prepare incident reports, advocate for individuals, and navigate documentation systems
  • Strong conflict resolution, communication, and organizational skills

Preferred Qualifications:

  • Current certifications in CPR/First Aid, CPI, and Defensive Driving

Physical & Work Environment

  • Regular lifting and carrying (up to 30 lbs), along with bending, reaching, standing, walking, and pushing/pulling
  • Work conducted in both facility and community-based settings
  • Some travel to other Claratel facilities may be required
  • Full-time, 40 hours/week | Standard schedule: Mon – Fri, 8:00 AM – 4:00 PM
    (Schedule subject to change based on program needs)
