Concordia Home Health and Hospice of Bethlen is hiring Hospice Nurses Registered Nurse (RN). Hospice Nurses and Registered Nurses are needed for patient care in and around Ligonier, allowing you to work close to your home. Graduate nurses are welcome to apply!

QUALIFICATIONS FOR THE Hospice Nurse - Registered Nurse RN position:

  • Graduate of state-approved school with course in pharmacology
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.

RESPONSIBILITIES for the Hospice Nurse Registered Nurse RN

  • Works with the Interdisciplinary Group (IDG) to develop, coordinate, and update the individualized plan of care for hospice patients/families cooperatively with the patient and/or family, physician, and hospice team
  • Coordinates and communicates with the hospice team the weekly patient schedule to meet the needs of the patient and family
  • Completes a supervisory visit on the Hospice Aide as required and oversees the appropriate implementation of the individualized plan of care by the Licensed Practical Nurse (LPN), Hospice Aide, and Volunteer
  • Conducts a physical examination and nursing assessment of the hospice patient at each skilled nursing visit
  • Serves as the primary nurse for patient case load coordinating the total patient care, evaluating the effectiveness and the appropriateness of the plan of care, making changes when necessary
  • Provides personal care such as (but not limited to): bathing, grooming, dressing, shaving, feeding; assessing vital signs; assisting with ambulation; changing position in bed; assisting with transfers, changing bed linens; assisting with elimination; providing oral hygiene, skin care, foot care, ear care, light house cleaning and meal preparation
  • Together with the hospice team, provides care as established by the IDG to meet the physical, emotional, and spiritual needs of the patient as necessary, and appropriately supports the family at the time of death
  • Completes all computer and/or paper documentation requirements within the timeframes specified
  • Participates in required on-call responsibilities
  • Attends and participates in IDG meetings, staff meetings and other related activities when required
  • Completes required in-services; ensures all required continuing education credits are obtained for licensure renewal
  • Demonstrates an understanding of the Quality Assessment Performance Improvement (QAPI) process, hospice policies and procedures, and participates in projects when requested
  • Participates in the orientation of new staff members when requested
  • Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and Concordia Home Health & Hospice of Bethlen policies and procedures; maintains confidentiality of patient and employee records and information; has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to the Executive Director and/or Compliance Manager.
