Concordia Visiting Nurses is seeking a Clinical Manager RN, PT, OT or SLP for a full-time position which will report into our Administrative office near Butler, PA.

The Clinical Manager is a licensed health care professional that supervises and coordinates staff to ensure provision of quality patient care and effective team performance to meet agency goals in accordance with philosophy, policies, mission and vision.

QUALIFICATIONS FOR THE CLINICAL MANAGER:

  • Registered nurse with a current valid Pennsylvania license with a current valid Pennsylvania license. BSN recommended. OR Therapist, physical therapist, occupational therapist, or speech-language pathologist, with a current valid Pennsylvania license. 
  • Two [2] years home health experience with roles reflective of progressive responsibility, including supervision in home health.
  • Knowledge base of agency policies, federal and state regulations and third-party payers
  • Exhibits strong organizational, communication and interpersonal skills. Evidence of leadership activities
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
  • Experienced with computers and electronic health record software/documentation.

DUTIES AND RESPONSIBILITIES for the Clinical Manager:

  • Coordinates staffing patterns to meet staff and geographical needs among staff; take and coordinate referrals as necessary to ensure agency coverage
  • Maintains schedules for all staff daily, including patient visit schedule, weekend, holiday, and on-call schedules.  Handles phone calls and messages to and from staff, patients, families, and referral sources
  • Accountable for clinical staff and provision of appropriate clinical services to patients serviced by the agency
  • Interviews applicants and assists in hiring decisions to maintain a qualified staff in sufficient numbers to serve patient needs
  • Assures that patient needs are continually assessed and assists with consulting physicians for orders and care plan development, implementation, and updates. Maintains clinical skills by providing hands on care to patients as required and attending annual competency fair
  • Applies problem solving abilities to specific organization/clinical activities
  • Works with the Director of Compliance and Education to assure regulatory and certification compliance at local, state, and federal levels
  • Participates in Utilization and Quality Improvement activities
  • Identifies need for in-services for the staff and assists with in-services, as needed
  • Provides back up to the Clinical Supervisor for workflow tasks, patient and referral phone calls and clinician education as needed Provides back up to the Branch Manager, as needed
  • Assists in supervising all patient care activities to assure compliance with current standards of accepted nursing practice
  • Assists with competency testing for clinical staff
  • Performs performance reviews of the clinical staff with input and review as needed from the clinical leadership team
  • Monitors and maintains productivity reports Works with the education team to ensure all staff receive proper onboarding and orientation
