Position Summary:
Responsible for providing specialized nursing services to patients and caregivers at home or in alternative settings. Provides technical nursing and skilled interventions as ordered by the physician. Instructs, motivates, and assists patients / caregivers in learning and improving ability to live independently at home.
Essential Functions and Job Responsibilities:
- Ensures physician orders are followed and included in patients plan of care
- Development and revision of the plan of care based on physician orders
- Ensures nursing orders are carried out in a professional manner satisfying professional ethics, patient rights and State and Federal codes and regulations
- Continually monitoring patient outcomes and reporting changes to supervisors to prevent hospital readmission
- Participates in effective communication amongst other clinicians providing care to the patient
- Completes and submits all required documentation within established agency guidelines
- Communicates the patient’s progress to supervisor and make changes as necessary under the physician’s direction
- Covers after hours, weekend and holiday on call to meet the branches need
- Maintains confidentiality of all company and patient related information. Protects patient rights as defined in the patient Bill of Rights
- Attends and participates in staff meetings, required in-services and other meetings as requested
- Instructs patients/caregivers of the safety measures appropriate to care and environment and observes/implements standard precautions and infection control for all patients.
- Ensures accurate patient assessments are completed timely
- Coordination of related services for patients
- Response to emergency patient needs timely and escalating to supervisors and physicians as needed
- Always represents self in a professional and ethical manner
- Takes professional responsibility for self-protection during required work in potentially hazardous situations
- Completes all required patient care documentation and reports accurately and in a timely manner
- Communicates with clinical staff, physicians, and other health care providers to assure the quality and continuity of patient care
- Responsibility and extended patient related travel as required by the service area
- Participates in the activities of the Performance Improvement Program.
- Develop and maintain working knowledge of current HME products and services offered by the company.
- Maintain patient confidentiality and function within the guidelines of HIPAA.
- Completes assigned compliance training and other educational programs as required.
- Maintains compliant with AdaptHealth’s Compliance Program.
- Perform other related duties as assigned.
Schedule: Monday through Friday, 8:30am-5pm with rotating Saturday on-call.

- Computer skills including knowledge of Microsoft Office applications
Education and Experience Requirements:
- Associate degree in nursing or equivalent
- Current and unrestricted LPN clinical license
- Two (2) years of clinical nursing experience is required
- Five (5) years of related HME experience is preferred
- Valid and unrestricted driver’s license
Physical Demands and Work Environment:
- Must be able to bend, stoop, stretch, stand, and sit for extended periods.
- Ability to perform repetitive motions of wrists, hands and/or fingers due to extensive computer use.
- Work environment may be stressful at times, as overall office activities and work levels fluctuate
- Subject to long periods of sitting and exposure to computer screen
- Excellent ability to communicate both verbally and in writing
- May be exposed to angry or irate customers or patients
- Must be able to drive and travel as needed
- Ability to utilize a personal computer and other office equipment
- Ability to demonstrate courtesy and respect for privacy
- Must be able to work extended hours when needed and drive for long periods as needed
