 Location: Jamestown, ND

 Shift: Varying hours

 Job Schedule: Full Time

 Weekly Hours: 32 (20 hours of being in the RN Care Coordinator position, 12 hours of on the floor nursing shift)

 

JOB SUMMARY:

 

The purpose of the RN Care Coordinator is to coordinate and oversee the medical/nursing, social and developmental needs of children with intellectual and/or development disabilities and medical complex needs for the individual’s overall plan.  He/she acts as an advocate for the individual and ensures completion of monitoring functions along with the other team members.  This position assumes authority, responsibility, and accountability for the coordination of services and programming for the individuals is carried out in conjunction with other members of the team using all aspects of information gathered.  The position reports to the Family Services Manager, however, with support from the Director of Nursing uses independent clinical judgment to provide clinical case management.

 

QUALIFICATIONS (minimum requirements)

Education and/or years’ experience required:

  • Graduate of an accredited school of nursing as a Registered Nurse, minimum of associate’s degree
  • Current and unencumbered RN license or eligible to achieve
  • One year experience in the care of medically complex children/individuals

 

Preferred Skills and Abilities:

  • Bachelor’s degree in nursing
  • Master’s degree in nursing or related business field
  • Two years’ experience in the care of medically complex children/individuals
  • Pediatric critical care experience preferred
  • Working knowledge of advanced technology and specialized equipment in the care of medically complex children/individuals

 

Supervision of others/decision making aptitude (if applicable): This is position is not responsible for supervision.

 

  • Demonstrates ability to complete a thorough evaluation of an individual served medical history, development status, social circumstances and family dynamics to identify needs and potential barriers to support and services
  • Demonstrates ability to understand and administer nursing care including advanced technology, specialty equipment, mediations, treatment, and the nursing process
  • Demonstrates ability to manage the care coordination of medically complex cases and prioritize effectively
  • Demonstrates ability to learn and direct education and therapeutic programs
  • Demonstrates ability to adapt to rapid change and manage emergencies as needed
  • Respects and advocates for the protection of each individual’s rights
  • Demonstrates ability to ensure medical/nursing services are coordinated and carried out in a timely manner to meet client’s needs, from admission thru discharge
  • Demonstrates ability to regularly review individual’s progress, adjusting care plan/program as needed and documents outcomes to assess effectiveness
  • Demonstrates ability to identify and connect families with relevant community resources when needed
  • Demonstrates ability to develop and monitor medical/nursing care, outcomes and evaluation of the client including making necessary adjustments to care when needed
  • Demonstrates ability to communicate, coordinate, facilitate and assist with specialty clinics and assist with specialty clinic visits to meet the medical needs of the clients 
  • Demonstrates ability to facilitate the transitions of care including discharge planning when needed
  • Demonstrates ability to effectively listen and communicate with students/clients, families, outside constituents and employees.
  • Participates in multidisciplinary team meetings, providing recommendations for care
  • Demonstrates ability to work with the Individual Program Coordinator to oversee program assisting with determination of adequacy in coordinating services to meet individual goals and satisfaction. 
  • Demonstrates ability to work with the nursing staff for ongoing monitor of medical needs
